Whitehall District Schools Athletics
Whitehall District Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign all athletic participation forms for your students, pay any associated athletic fees, along with many more time-saving features. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
In addition, you will be able to print the MHSAA physical examination form directly from your FinalForms site and take that with you to the doctor for your child’s physical (no need to pick one up from school). Athletes must have a valid physical performed on or after April 15 of the previous school year in order to be eligible to participate. Please upload the physical form to FinalForms when completed by you, the doctor, and your student athlete.
You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update. You will also be required to upload the physical form and pay the athletic fee once per year. Click on the Parent Playbook (attached) to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”.
We are asking that ALL parents of Whitehall HS and Whitehall MS athletes register their athletes on FinalForms. Please register at: Final Forms and follow the prompts to create your account, create your students, sign your forms, upload the physical form, and pay the $50 or $75 athletic fee.
We believe that Whitehall District Schools' athletes should compete honorably while representing their team, school and community. With appropriate attitude, dedication, and leadership, they will contribute positively to school spirit and community pride. To accomplish this, we encourage the cooperation of athletes, parents, and boosters. Participation in athletics is a privilege and when student-athletes participate on an athletic team, those individuals assume the responsibility of being team members.
The responsibilities include:
- Being aware that he/she will be expected to act as a positive force in the school.
- Follow the code of conduct in the athletic code of ethics and student handbook.
- Complying with team rules and policies.
- Maintaining academic eligibility (pass four core classes/1.5 or above GPA).
- Maintaining a positive attitude.
- Understanding his/her role as a team member
