Printable MHSAA Physical Form - please take this with you to your child's pre-participation physical
Interscholastic participation begins in grade six for cross country, wrestling, and track. Participation begins in grade seven for all other sports at the middle school level. Whitehall District Schools has adopted a Pay to Participate fee which requires all Middle School Students to pay a fee of $50.00. Your child is required to pay this fee before the first practice. You may pay online by visiting the Athletics tab from the main page, and selecting PaySchools.
Please view the Athletic Information section (pages 10 - 12) in our Handbook. To participate in sports all students must have a MHSAA physical form (see link above) completed by a physician and parent/guardian. This completed form must be turned in to the office on or before the first day of practice. One physical will cover your child for the entire school year.
Students are also required to complete the ImPACT Baseline Testing with our Athletic Trainer each year. You may read the letter by clicking on the link ImPACT for dates and times that they may test.
